Update on epinephrine: Excessive cost is leading to increased errors

Epinephrine auto–injectors have become cost prohibitive for many institutions to stock, resulting in an increased use of epinephrine ampuls or vials to manage anaphylactic reactions. Unfortunately, use of these ampules and vials in the hospital setting has resulted in increased medication errors, such as administration of the wrong dose or via the wrong route. The Institute for Safe Medication Practices (ISMP) released a safety alert informing clinicians that use of epinephrine ampuls and vials may be a cheaper method to manage anaphylaxis, but it introduces a much higher risk of errors, with some resulting in fatalities.
